To make an appointment, please email us at service@cchc.org.tw or add line  and provide the following information: 

• A photocopy of your passport

• Personal health information, including your current diagnosis

• Any films or biopsy results related to your condition (X-ray, MRI, CT scan, ultrasound)

• The name of the department you wish to visit

• A list of questions you have for your doctor

• Your contact information, including your mailing address, email address, and phone number

• Or you can fill the Inquiry Form

Step 2: Receive Your Appointment Confirmation

We will confirm your appointment shortly after receiving your information and completing the new patient registration. We will also provide you with all the information you need before traveling to our clinic.
